In order to participate in any athletic activity, your child is required to complete and submit the following forms:

• AHSAA Physical Form
• AHSAA Concussion Information and Forms
• AHSAA Student/Parent Sudden Cardiac Arrest Form
• TCS HIPPA Form
• TCS Permission and Insurance Verification Form
